Job Summary:
Responsible and accountable for providing safe, effective, and promptmedication therapy to hematology/oncology patients.

Develops and/or maintains an active clinicalpharmacy practice on the assigned service(s); participates actively in roundsand/or outpatient clinic coverage (including assigned on-call and weekendcoverage).
Assesses and evaluates medical and drug literature critically;develops/leads formulary management, medication therapyrecommendations, and optimization initiatives.
Participates in qualityassurance (QA) activities (e.g., drug utilization evaluations, qualityimprovement (QI) initiatives, and evaluating clinical pharmacists’ impact ondrug therapy measurements).
Establishes and/or maintains a viable practicesuitable for educating and training.
Performs Pharmacist duties (asrequired).
Performs other duties.

None. Oversees the work of, but does not directly supervise, the following:Pharmacy Technician I/Non-Cert (JC: 072205), Pharmacy Technician II/Cert(JC: 072305), Pharmacy Technician III/Cert (JC: 072405), and PharmacyTechnician IV/Cert (JC: 072705).
Required Education:
Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited school of pharmacy
Preferred Education:
Required Experience:
One (1) year of relevant registered pharmacist work experience
Preferred Experience:
Required Certification/License/Registry:
Licensed [or eligible for licensure (i.e., meets all statutory requirements for licensure application)] as a pharmacist in accordance with Florida Statutes (F.S.), Title XXXII (Regulations of Professions and Occupations), Chapter 465 (Pharmacy), Sections 007 through 013. Basic Life Support (BLS) certification from an American Heart Association (AHA) or American Red Cross (ARC) recognized provider. Board of Pharmacy Specialties (BPS) Board Pharmacy Specialty Certification (BCOP) and/or BPS Board Certified Pharmacotherapy Specialist (BCPS) credential(s).
